The Jo Ann and William J. Dinwiddie Education Scholarship Application

Scholarship Requirements:

Application Deadline:
• Online application form and photo must be submitted by April 1st.
• All additional required application materials must be postmarked no later than April 1st.

Official Transcript:
• High School Seniors: You must submit an official high school transcript. (A copy bearing the official stamp of the school may be sent from the school office.)
• College Freshmen or First-year Vocational Students (at time of application): You must submit an official high school transcript. (You may also submit an official college/vocational school transcript in addition to the high school transcript.)
• College Sophomores, Juniors & Seniors or Second-year Vocational Students (at time of application): You must submit an official college/vocational school transcript only.

Standardized Test Score (ACT or SAT):
• High School Seniors: Test score may appear on an official high school transcript or be sent to the Foundation directly from the ACT or SAT services.
• College Freshmen or First-year Vocational Students (at time of application): Test score may appear on an official high school transcript or be sent to the Foundation directly from the ACT or SAT services.
• College Sophomores, Juniors & Seniors or Second-year Vocational Students (at time of application): A test score is not required.

Tipton Confirmation Form:
The applicant must take the Tipton Confirmation Form to either the Tipton UMC pastor or their counselor at Tipton High School to complete and mail to the Foundation (PO Box 1076, Columbia, MO 65205) indicating the applicant is either a member of the Tipton UMC or a graduating student of Tipton High School.

Before you begin filling out the online application:
• You must complete and submit the application all at one time. You will NOT be able to save the application and come back. If you quit before completing the application, you will have to start over again.
• If you submit your application and determine changes or additions need to be made, please call the Foundation office at 800-332-8238. Changes and additions cannot be made online.
• You will receive an email confirming our receipt of your application once it’s submitted. Please retain the confirmation email for your records. If you do not receive an email, please call our office. Your application may not have been received by the Foundation.

This application and related materials are submitted for consideration only for the academic year noted above, and the information contained in these materials is accurate and truthful to the best of my knowledge. I have not received scholarship assistance from the Missouri United Methodist Foundation (“Foundation”) in prior years.

The scholarship decision is within the sole discretion of the Foundation. All scholarship decisions are final and are not subject to further review or contest by any student applicant. Scholarship awards are made contingent on verification of enrollment.

In the event that I receive a scholarship or grant of financial assistance from the Foundation, I hereby authorize the Foundation to publish my name and picture in various public media in order to publicize the Foundation scholarship and grant programs and to acknowledge my award.
